• Two oral diagnosis
• Two oral cancer screenings
• Two cosmetic consultations
• One emergency visit with single X-ray (220,140)
  
• One fluoride treatment (children- up to age 14) (1206,1208)
  
• 15% discount on sealant treatment (children)
  
• 15% discount on all restorative services (excluding dentures)
  
• 15% discount on panoramic x-ray and cone beam scans
• 15% discount on in-office whitening or home whitening kits
  
• 10% discount on dentures and rebase

Provisions of Patient Loyalty Plan

• Payment is due at the first day of treatment.
• Cannot be combined with other insurance plans, discount plans, or promotions, including using CareCredit.
• The Plan is for you and is not transferable.
• Benefits are provided for one year from the date of the Plan is purchased.
